BT Asset Pre-Check Submission – GDPR Notice

This form is for use by BT Authorised Suppliers to request asset availability checks through Netify. It replaces the previous spreadsheet/email process to improve data handling and ensure GDPR compliance.

Under the UK General Data Protection Regulation (GDPR):

You must gain customer authorisation before submitting this request.
You must also gain authorisation again before sharing the asset results with the customer.
You must retain recorded proof of consent (e.g., call recording or written confirmation) and supply it to Netify or BT on request.

It is your responsibility to ensure that all submitted data has been lawfully collected and that consent is properly recorded.

If you or your customer have any concerns about how personal data is being handled, you can contact the Information Commissioner's Office (ICO), the UK’s independent data protection authority.

DO NOT submit this form unless you have full customer authorisation for both requesting and returning BT asset data.

Data Storage and GDPR Compliance

Under the UK General Data Protection Regulation (GDPR), any personal or business data collected during the pre-check process must be stored securely and only for as long as necessary to fulfil its purpose.

By submitting this form, you confirm that:

All data collected is relevant to the BT asset pre-check process.
Customer details and consent records will be stored securely within your organisation.
Access to this data is restricted to authorised personnel only.
Data will not be used for any other purpose or shared outside of the BT pre-check process.

You must be able to provide proof of consent and justification for storing any customer data if requested by Netify, BT, or the Information Commissioner's Office (ICO).

All data must be handled in accordance with GDPR principles of lawfulness, transparency, and accountability. If you are unsure about your responsibilities, visit the ICO website for guidance.
